Start your NHS Health Check booking by checking your eligibility. If you are unsure of the answer to any of the questions below, please contact your GP to confirm prior to making your booking.

Eligibility
Are you aged 40-74 and live in Norfolk or registered with a Norfolk GP?
Your Health
Have you been diagnosed with any of the following?

  • Coronary Heart Disease
  • Chronic Kidney Disease (Stage 3–5)
  • Diabetes (Type 1 or 2)
  • Hypertension (High Blood Pressure)
  • Atrial Fibrillation (heart condition that causes an irregular and often abnormally fast heart rate)
  • Stroke or Transient ischaemic attack (TIA - or "mini stroke" is caused by a temporary disruption in the blood supply to part of the brain)
  • Familial Hypercholesterolemia (Inherited high cholesterol – defined as familial hypercholesterolemia)
  • Heart Failure
  • Peripheral Arterial Disease

Previous checks and medication
Are you currently being prescribed statins to lower cholesterol or have been identified as having a 20% or higher risk of developing cardiovascular disease over the next 10 years?
